Hip fractures in elderly" This is a very common and important fracture in geriatric age group.As our life expectancy is increasing the osteoporosis burden is also very high.This leads to fractures arround hip which is a vulnerable area. Fracture neck of femur and inter-trochantric fractures are classified according to exact location of it as it helps in decision making. When we have such fractures usually they are managed with surgery as our aim is to make the patient upright and mobile as early as possible.When a person is bed ridden the chances of survival is very less due to increased chances of chest infection, DVT, bed sores and decrease in cardiac function. The patient has to undergo few tests(blood, urine, Chest x ray and ECHO )to know the possible risks and we are prepared to mange them. Type of surgery: 1.Fracture neck of femur:As this area is less vascular so chances of union is very minimal so we replace the head of femur with a metal known as hemi-replacement(as cup is not changed). Patient can made to walk full weight bearing from next day. 2.Inter-trochantric fractures:have good vascular supply so can aim union Can be manged by a) Proximal femoral nailing/Dynamic hip screw fixation b)bipolar hemi-replacement.We don't want to wait for union. Post-operative physiotherapy and care is also important to mobilise the patient out of bed as early as possible.
